Medical insight: We consulted a medical expert to better understand the benefits of a medium-firm mattress and why this firmness level is often recommended for people with back pain. Research: We considered mattresses from reputable brands featuring a variety of materials and constructions to accommodate different sleep styles and preferences. We also analyzed hundreds of customer reviews, prioritizing models with average ratings of 4.5 stars or higher. What To Consider When Shopping For A Medium-Firm Mattress According to Samantha Parker Lane, M.D., neurosurgeon at Mischer Neuroscience Associates, “A medium-firm composition offloads pressure from sore joints while still providing support.” This profile can work well for most sleeping positions, all body types, and those seeking specific benefits, like pressure-relief, breathability or motion isolation. However, medium-firm mattresses aren’t one-size-fits all. Here’s what to consider when searching for your ideal medium-firm bed.

Mattress Types The composition and construction of a mattress affects how it feels, so it’s important to consider your preferences as you consider different medium-firm beds. A variety of materials can impart a medium-firm feel, and these materials also have specific benefits and drawbacks.

Hybrid mattresses combine innerspring coils with foam layers to balance support and comfort. Medium-firm beds in this category make sleepers feel lifted yet cushioned in key areas, like the lower back, shoulders and hips. While they can be pricer than medium-firm memory foam mattresses, they tend to be more durable (thanks to their coil construction) with better airflow. Memory foam mattresses are made of layers of poly foam to deliver a slow-moving, hugging sensation. Medium-firm mattresses with this construction deliver excellent pressure relief and motion isolation, making them a top choice for side sleepers and couples who don’t want to feel their partner’s movement. However, foam does retain more heat, and it’s oftentimes not supportive enough for heavyweight sleepers. Latex mattresses provide a similar pressure-relieving sensation as memory foam, with the added benefit of natural breathability and a more responsive feel. In other words, medium-firm latex mattresses are bouncy and responsive, making them a solid option for combination sleepers who switch between positions. Sleeping Position Medium-firm mattresses can be a great choice for back and combination sleepers. They should help you maintain proper spinal alignment while still offering some cushion around the hips and low back. Side sleepers often prefer a softer mattress that doesn’t skimp on spinal support and prevents pressure from building around the hips and shoulders. Depending on their body type, some stomach sleepers may find medium-firm mattresses are firm enough to properly elevate the hips.

Body Type A mattress can feel firmer or softer depending on your body type, so you may need to opt for a different firmness rating if you’re angling for a medium-firm feel. Petite sleepers, for example, could find that medium-firm options feel like a true firm, while a heavy sleeper over 200 pounds may consider them to be too cushioned for comfort. Spinal Alignment No matter what position you sleep in or what your specific needs are, spine alignment is key; it ensures your spine stays in its natural posture, which limits pressure on your lower back and joints. Medium-firm mattresses tend to excel at providing spine alignment because they provide the right amount of contouring (a.k.a. softness) and support. According to Lane, “The spine has a natural curvature to it that resembles two ‘s’ stacked together. Mattresses that are too soft don’t support the natural curvature of the spine, which can lead to pain. Conversely, mattresses that are too hard can create pressure points, which can also cause pain.” As a result, medium-firm mattresses that land in the middle of the spectrum provide the best of both worlds.

Lane also cites a randomized, double-blind and controlled study by Kovacs, which followed 3src3 adults who had chronic low back pain to assess the effect of mattress firmness on back pain. She says “they found that patients with medium-firm mattresses had significantly less pain on rising, daytime pain, and pain while lying compared to those on soft or very firm mattresses.”

Temperature Regulation Firmness is important whether you’re looking for a cooling mattress or not, but it also can play a role in how warm or cool a bed feels. The softer a bed is, the more you sink into it, which means your body retains more heat. Firmer beds, by contrast, tend to seem cooler since you’re on top of the mattress, not in it. Medium-firm picks can hit the sweet spot of both for a comfortable, supportive feel.
